ADVANCED DIRECTIVES

Upon admission to Northwest Medical Center, each patient is given a brochure regarding medical advance directives. If you would like information before you arrive at the facility, please contact the Admitting office during regular business hours.

DEPOSITS/COPAYMENTS

Patients are responsible for all financial liabilities including co-payments, deductibles, and out-of-pocket expenses at the time of registration or at the time of discharge for emergency admissions. If the exact dollar amount has not been determined, you will be asked to pay the estimated amount and will be billed for the remainder.

PRE-REGISTRATION

Pre-registration is available to all scheduled patients prior to the date of service to expedite the check-in process. During the pre-registration process, all registration information is obtained over the telephone. When you arrive at the facility, you will be required to provide your insurance cards and sign the required forms before going to the department.

MATERNITY ADMISSIONS

Pre-registration forms are available through our Admitting department or through your OB/GYN's office. These forms gather basic demographic and financial information and allow us to ensure that you are pre-registered and authorized for services prior to your admission.

Pre-registration forms should be completed at the beginning of the second trimester or at the time you select Northwest Medical Center for your delivery. Completed forms can be mailed or dropped off at the Admitting Department.

The following steps will help to ensure that your registration is performed in an efficient manner:

  • Before you arrive at the facility for tests or procedures, you may want to contact your insurance company to obtain pre-certification and benefits information.
  • Please verify with your physician's office that they have obtained the necessary authorization and referrals for your tests or procedure. The physician office must initiate the process of authorization.
  • You may be required by your insurance company to notify your Primary Care Physician that you will be treated at Northwest Medical Center.
